Its objectives are to search for, prevent and resolve problems related to medications, to try to achieve the expected health results, and maintain or improve the patient's quality of life.

Pharmaceutical care is an innovative concept in pharmacy practice, which emerged in the mid-1970s. It establishes that all healthcare professionals must take responsibility for the results of pharmacotherapy in their patients. It encompasses a variety of services and functions—some new to pharmacy, others traditional—that are determined and provided by pharmacists caring for individual patients.

The concept of pharmaceutical care also includes the expressed commitment to the well-being of patients as individuals who require and deserve the consideration, concern, and trust of pharmacists.

However, pharmacists often cannot accept responsibility for this expansion of patient care. Consequently, they may not adequately document, monitor and review the assistance provided. Accepting such responsibility is essential to the practice of pharmaceutical care.
